In the event of a work-related injury or illness. The Central Office has the workers' compensation information and the First Report of Injury reporting forms.
Please notify your supervisor immediately if you have an injury or accident while on duty.
Employees must also contact the Central Office at (940) 969-5001 within 24 hours to report a claim.
